Many women tell me the same thing: they have heard that PCOS (polycystic ovary syndrome) resolves on its own at menopause, and they have assumed that once periods stop, the problem stops too. But what I see in the clinic is different. A story that begins in the younger years with irregular periods can continue years later through metabolism and menopause; the end of menstruation is not an ending but a new phase of the same biology. That is why I look at PCOS not merely as a problem of the reproductive years, but as one of the early biological signals that help us understand a woman’s lifelong health.
Does PCOS disappear after menopause?
The short answer is no. PCOS does not disappear at menopause; only its visible face changes. In menopause, irregular periods vanish because ovulation has already stopped, so the syndrome can look as if it is “over.” Because PCOS reaches across the whole of a woman’s biology, in scientific circles it is now also referred to by a broader name — PMOS (polyendocrine metabolic ovarian syndrome).
This misconception is common, because for many years PCOS was seen only as a fertility problem. Yet irregular periods were just one symptom of the syndrome, not its root. At its core, insulin resistance, hormonal imbalance, and a metabolic tendency continue after menopause. The easing of symptoms is not the same as the syndrome resolving; assuming it has “gone” can let an ongoing metabolic risk slip out of view. What changes is not the syndrome itself, but its visible symptoms.
How do hormones change during menopause?
At menopause, the ovaries’ estrogen production drops markedly; this decline happens in all women. In those with a history of PCOS, a different balance sets the picture apart. The carrier protein that binds hormones in the blood (SHBG) tends to fall with menopause; as the carrier decreases, the proportion of free male hormone (androgen) in the blood rises relatively.
For this reason, some women may continue to have excess hair growth or skin-related signs after menopause. The relative rise in free androgen does not concern the skin alone; androgen and insulin feed one another, and when one increases, the other is strained too. Insulin is the hormone that carries blood sugar into the cells; when resistance develops, the cells respond less to this signal, and the body secretes more insulin to keep its balance. This tendency begins years earlier in PCOS and does not disappear at menopause.
The fall in estrogen strains insulin sensitivity further; fat around the abdomen increases, and blood sugar and lipid levels tend to rise. Some of this fat can also accumulate in the liver and strain the metabolic balance still more. A low-grade, silent inflammatory tendency can accompany it as well. Over time this picture carries the risk of metabolic syndrome and type 2 diabetes together; a history of PCOS forms a ground on which this risk quietly builds.
